Summary

A total of 64 DP incidents are detailed in this document. All the reports have been provided by IMCA members on the basis that the vessel name, owner and client at the time are kept confidential. Of the 64 incidents that were analysed, 10 of them were considered to be of the serious Loss of Position 1 type, 19 to be Loss of Position 2 incidents and the remainder lost time incidents.

One of the main categories for both Loss of Position 1 and Loss of Position 2 incident initiating events was found to be position reference related. This cause was the highest for all incident types reported to IMCA. Looking back over past data, position references have been one of the main initiating events for DP incidents.

Computer related incidents in 2002 were lower, in percentage terms, than reported in the previous two years. Generator related incidents were found to have increased in 2002.

The various initiating events for DP incidents is dependent on the data received. Generally the numbers of serious DP incidents has been reducing even though there is an increasing number of new DP vessels.
