Lessons learned and safety issues identified from the analysis of Marine Safety Investigation Reports submitted to IMO

A working Group on ‘Lessons Learned and Safety Issues Identified from the Analysis of Marine Safety Investigation Reports’ met during the 10th session of IMO’s Implementation of IMO Instruments (III) Sub-Committee which IMCA attended.

The Group considered 34 cases reported by IMO Member States which are reflected in this Information Note, and reported on:

  • the category of safety issues
  • the type of marine casualty or accident
  • the severity of the incident
  • what happened
  • why it happened, and
  • what can be learned and who would benefit from the information.
